Centralized Admissions Office

The Key to Effective Admission Process Execution

The primary function of the Centralized Admissions Office is to evaluate, refer and admit patients or residents for appropriate placement based on the individuals needs and current bed availability.

The Centralized Admission team receives referrals by phone, care referral partners, EMR, or by fax. Our staff will gather information about the needs of the perspective patient or resident and seamlessly refer inquiries to the appropriate Geer admissions staff for discovery and admission.

At Geer, an in-depth knowledge of regulatory laws and a full understanding of any medical condition of the patient is key to the effective admission process.

Referrals and Admissions

Referrals and admissions are accepted from an inpatient setting, such as a hospital, skilled nursing home, an emergency department, a doctor’s office, or from home.

Admissions are accepted 24 hours a day, 7 days a week.

Levels of care at Geer Nursing and Rehabilitation include:

  • Skilled
  • Rehabilitation
  • Dementia

Levels of care at The Lodge include:

  • Independent
  • Assisted Living
  • Dementia
  • Specialty Personalized Neighborhood

Our Centralized Admission process also does our best to accommodate the individual’s mind, body, and spirit and if we cannot meet these requirements, we do not admit.

Expectations and Role of Admissions Department

Our admissions team operate under the following core values:

  • Admissions’ role is to admit appropriately.
  • We admit the resident using all policies and procedures and within the mandates and guidelines of the law.
  • Admissions work with every department.
  • Admissions is to provide superior customer service to our outside referral sources and internally this is not an option, it is an expectation.
  • Admissions is the messenger to our referral sources.
  • Admissions look to find ways to admit, not decline.
  • Admissions understands no one asks to be ill or to need our services and we act as a resource regardless of where a patient decides to go.
  • Admissions is an outcome-driven model and what we do is measurable every day.
